Service Description

Having trouble with discharge planning for yourself or a loved one? Do you need help setting up your home as a place to care for a someone who is unwell? We can help, guiding you in finding community resources and funding to access equipment and home modifications to make your home safe for your mobility status.

Cancellation Policy

If you need to cancel or re-schedule please contact us at least 24 hours before your appointment. Failing to do so may result in a missed visit fee of no more than the cost of the booked service.
